The difference is that you either Run the executable to install it directly from the Temp directory or if you let it ''Save'', it will save the file you are downloading to the directory you chose so you can execute it from whereever on your hard drive you want.
I always choose save it then run it from my computer, that way if you ever need it again it will be on your computer already and you wont have to download it again.



If you choose run, it will throw it in a temp directory and run it from there. sometimes it stays there and sometimes it doesnt depends if you clean out your temp internet files or not.
if you trust the site and it as a publisher,run it or you could save it and decide to run it or not at a later date
Install Linux and stop worrying about malware.

save it allows you to run or use the program whenever you want opposed to just that one time
Always best to save the file and scan with anti-virus and anti-malware unless you know for 100% the software is safe.
